Case Manager - intake and assessment (Salt Lake City, UT)

Stonegate Lakeside Care Services is a social-care provider serving older adults and people with disabilities around Salt Lake City. We are hiring a Case Manager to take on intake and assessment. The work is personal and so is the culture. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for visit scheduling. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Salt Lake City. • We reply to every applicant.
