Case Manager - intake and assessment (Sacramento, CA)

Westbrook Technologies is a healthcare provider in Sacramento with clinics and support teams. We are hiring a Case Manager to take on intake and assessment. You would join a large organisation of about 546 people based in Sacramento. What you will do • Help with applications and appointments for patient education. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Full-time, Monday to Friday. • On site in Sacramento, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
