Case Manager - intake and assessment (Sacramento, CA)

We are growing our team at Northwind Birch Care Services and adding a Case Manager whose main responsibility will be intake and assessment. We train and promote from within. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for visit scheduling. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Sacramento. • We will tell you what to expect at each step of the hiring process.
