Case Manager - intake and assessment (Sacramento, CA)

We are growing our team at Fernwood Ivory Health and adding a Case Manager whose main responsibility will be intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for patient scheduling. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Sacramento, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
