Case Manager - intake and assessment (Sacramento, CA)

Highbridge Meridian Health Group provides primary and specialty care in Sacramento, and we need a Case Manager to lead our intake and assessment. Clinicians and administrators work side by side. What you will do • Help with applications and appointments for clinic supplies. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, spread across three days. • On site in Sacramento, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
