Case Manager - intake and assessment (Sacramento, CA)

Lumen Wren Care Services supports families and individuals in Sacramento with personal care and coordination. We are hiring a Case Manager to take on intake and assessment. You would join a large organisation of about 756 people based in Sacramento. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for care plans. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Sacramento. • We reply to every applicant.
