Case Manager - intake and assessment (Richmond, VA)

Clearpath Vista Community Partners is looking for a Case Manager. The job centres on intake and assessment for grant reporting, and it would suit someone who likes steady, practical work. You would join a large organisation of about 617 people based in Richmond. What you will do • Help with applications and appointments for grant reporting. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with grant reporting.
