Case Manager - intake and assessment (Richmond, VA)

Westbrook Technologies runs clinics and patient services around Richmond. We are hiring a Case Manager to take on intake and assessment. Continuing education is supported. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for referrals. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with referrals. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in Richmond. • We will tell you what to expect at each step of the hiring process.
