Case Manager - intake and assessment (Reno, NV)

Stonegate Yarrow Care Services Group provides in-home and community support in Reno. We are hiring a Case Manager to take on intake and assessment. The work is personal and so is the culture. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for licensing. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Contract for four months, with the possibility of becoming permanent. • On site in Reno, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
