Case Manager - intake and assessment (Reno, NV)

Northwind Birch Care Services provides in-home and community support in Reno. We are hiring a Case Manager to take on intake and assessment. Teams support each other, especially on hard days. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for care plans. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Full-time, Monday to Friday. • On site in Reno, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
