Case Manager - intake and assessment (Reno, NV)

We are growing our team at Stonegate Yarrow Care Services Group and adding a Case Manager whose main responsibility will be intake and assessment. The work is personal and so is the culture. What you will do • Help with applications and appointments for visit scheduling. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Contract for twelve months, with clear deliverables from the first week. • On site in Reno, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
