Case Manager - intake and assessment (remote)

Summit Field Community Partners is looking for a Case Manager. The job centres on intake and assessment for donor records, and it would suit someone who likes steady, practical work. Mission matters here, and so does looking after staff. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for donor records. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with donor records. Working arrangement • Part-time, about 28 hours a week, spread across three days. • Remote-friendly, with quarterly visits in Charleston. • We will tell you what to expect at each step of the hiring process.
