Case Manager - intake and assessment (remote)

Lumen Vista Housing Group is looking for a Case Manager. The job centres on intake and assessment for waiting lists, and it would suit someone who likes steady, practical work. Collaboration with local agencies is constant. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for waiting lists. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with waiting lists.
