Case Manager - intake and assessment (remote)

Rivermark Mill Care Services Partners is looking for a Case Manager. The job centres on intake and assessment for caregiver training, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for caregiver training. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with caregiver training. • Bilingual English and Spanish.
