Case Manager - intake and assessment (remote)

We are growing our team at Rivermark Vista Community Partners and adding a Case Manager whose main responsibility will be intake and assessment. You would join a small organisation of about 20 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for food distribution. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with food distribution. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, mornings. • Fully remote, with quarterly visits in Toledo. • Applications are reviewed on a rolling basis.
