Case Manager - intake and assessment (remote)

Silverline Corner Community Partners runs community programmes across Chicago. We are hiring a Case Manager to take on intake and assessment. Flexibility and kindness are expected. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for client intake. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with client intake. • Bilingual English and Spanish. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Fully remote, with an occasional day on site in Chicago. • We will tell you what to expect at each step of the hiring process.
