Case Manager - intake and assessment (remote)

We are growing our team at Silverline Sable Community Partners and adding a Case Manager whose main responsibility will be intake and assessment. Mission matters here, and so does looking after staff. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for benefits enrolment. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with benefits enrolment. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, with flexible days. • Fully remote, with a monthly team day in Detroit. • We reply to every applicant.
