Case Manager - intake and assessment (remote)

We are growing our team at Foxglove Cobalt Learning Group and adding a Case Manager whose main responsibility will be intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for family communication. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with a monthly team day in Dallas. • We will tell you what to expect at each step of the hiring process.
