Case Manager - intake and assessment (remote)

Clearpath Bright Community Partners is a nonprofit in Atlanta supporting families with food, housing and job help, and we need a Case Manager to lead our intake and assessment. You would join a small organisation of about 40 people, working mostly from home. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for benefits enrolment. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with benefits enrolment. Working arrangement • Full-time, on a regular day shift. • Fully remote, with quarterly visits in Atlanta. • We reply to every applicant.
