Case Manager - intake and assessment (remote)

Highbridge Cedar Health Group is a healthcare provider in Milwaukee with clinics and support teams. We are hiring a Case Manager to take on intake and assessment. You would join a small organisation of about 36 people, working mostly from home. What you will do • Help with applications and appointments for electronic records. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Full-time, Monday to Friday. • Remote-friendly, with a monthly team day in Milwaukee. • We will tell you what to expect at each step of the hiring process.
