Case Manager - intake and assessment (remote)

Silverline Granite Housing manages affordable housing communities across Jacksonville. We are hiring a Case Manager to take on intake and assessment. You would join a small organisation of about 19 people, working mostly from home. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for lease compliance. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with lease compliance. Working arrangement • Contract for eight months, with clear deliverables from the first week. • Remote-friendly, with an occasional day on site in Jacksonville. • Applications are reviewed on a rolling basis.
