Case Manager - intake and assessment (remote)

Rivermark Hollow Care Services is a social-care provider serving older adults and people with disabilities around Pittsburgh, and we need a Case Manager to lead our intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for licensing. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Contract for four months, with the possibility of becoming permanent. • Remote-friendly, with a monthly team day in Pittsburgh. • Applications are reviewed on a rolling basis.
