Case Manager - intake and assessment (remote)

Rivermark North Learning Group is an education provider in Madison serving students and families, and we need a Case Manager to lead our intake and assessment. Families are partners. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for learning materials. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with learning materials. • Bilingual English and Spanish. Working arrangement • Contract for twelve months, with clear deliverables from the first week. • Fully remote, with an occasional day on site in Madison. • We reply to every applicant.
