Case Manager - intake and assessment (remote)

Cedar Lakeside Health Group is looking for a Case Manager. The job centres on intake and assessment for insurance authorisations, and it would suit someone who likes steady, practical work. You would join a large organisation of about 891 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for insurance authorisations. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with insurance authorisations. • Bilingual English and Spanish.
