Case Manager - intake and assessment (remote)

Greenridge Linden Health Partners provides primary and specialty care in San Jose, and we need a Case Manager to lead our intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for electronic records. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Contract for nine months, with the possibility of becoming permanent. • Remote-friendly, with a monthly team day in San Jose. • We reply to every applicant.
