Case Manager - intake and assessment (remote)

We are growing our team at Bluepeak Meadow Early Learning and adding a Case Manager whose main responsibility will be intake and assessment. You would join a large organisation of about 456 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for family communication. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Contract for eight months, with clear deliverables from the first week. • Fully remote, with quarterly visits in Portland. • We reply to every applicant.
