Case Manager - intake and assessment (remote)

Foxglove Ivory Health Partners provides primary and specialty care in Hartford, and we need a Case Manager to lead our intake and assessment. We are busy and kind, and we look after each other. What you will do • Connect clients with partner agencies. • Help with applications and appointments for insurance authorisations. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Part-time, about 16 hours a week, afternoons. • Fully remote, with quarterly visits in Hartford. • We will tell you what to expect at each step of the hiring process.
