Case Manager - intake and assessment (remote)

We are growing our team at Redstone Forge Care Services Partners and adding a Case Manager whose main responsibility will be intake and assessment. We train and promote from within. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for licensing. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with licensing. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with an occasional day on site in Buffalo. • We will tell you what to expect at each step of the hiring process.
