Case Manager - intake and assessment (remote)

We are growing our team at Amberfield Ridge Early Learning and adding a Case Manager whose main responsibility will be intake and assessment. Low staff turnover is a goal we take seriously. What you will do • Connect clients with partner agencies. • Help with applications and appointments for meal programmes. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with meal programmes. • Bilingual English and Spanish. Working arrangement • Contract for three months, with clear deliverables from the first week. • Fully remote, with quarterly visits in Hartford. • We will tell you what to expect at each step of the hiring process.
