Case Manager - intake and assessment (remote)

Rivermark Vale Property Management Partners looks after rental homes across the Toledo area, and we need a Case Manager to lead our intake and assessment. You would join a small organisation of about 17 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for resident events. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with resident events. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with an occasional day on site in Toledo. • We reply to every applicant.
