Case Manager - intake and assessment (remote)

We are growing our team at Stonegate Labs and adding a Case Manager whose main responsibility will be intake and assessment. Mission matters here, and so does looking after staff. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for donor records. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with donor records. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Fully remote, with a monthly team day in Tulsa. • We will tell you what to expect at each step of the hiring process.
