Case Manager - intake and assessment (remote)

Highbridge Quill Health Group runs clinics and patient services around Charleston, and we need a Case Manager to lead our intake and assessment. You would join a small organisation of about 25 people, working mostly from home. What you will do • Help with applications and appointments for patient education. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with patient education. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Remote-friendly, with a monthly team day in Charleston. • We reply to every applicant.
