Case Manager - intake and assessment (remote)

We are growing our team at Fernwood Pine Housing and adding a Case Manager whose main responsibility will be intake and assessment. You would join a small organisation of about 92 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for lease compliance. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with lease compliance. Working arrangement • Contract for twelve months, with the possibility of becoming permanent. • Fully remote, with quarterly visits in Toledo. • We will tell you what to expect at each step of the hiring process.
