Case Manager - intake and assessment (remote)

Bluepeak Hollow Health Group is a healthcare provider in Boston with clinics and support teams, and we need a Case Manager to lead our intake and assessment. You would join a small organisation of about 192 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for patient education. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Part-time, about 20 hours a week, afternoons. • Remote-friendly, with a monthly team day in Boston. • Applications are reviewed on a rolling basis.
