Case Manager - intake and assessment (remote)

Bluepeak Cedar Health runs clinics and patient services around Salt Lake City, and we need a Case Manager to lead our intake and assessment. Clinicians and administrators work side by side. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for insurance authorisations. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Full-time, on a regular day shift. • Fully remote, with quarterly visits in Salt Lake City. • We will tell you what to expect at each step of the hiring process.
