Case Manager - intake and assessment (remote)

Northwind Glen Health is a healthcare provider in Detroit with clinics and support teams. We are hiring a Case Manager to take on intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for insurance authorisations. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Contract for six months, with clear deliverables from the first week. • Fully remote, with quarterly visits in Detroit. • Applications are reviewed on a rolling basis.
