Case Manager - intake and assessment (remote)

Westbrook Linden Health is looking for a Case Manager. The job centres on intake and assessment for patient scheduling, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for patient scheduling. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling. Working arrangement • Part-time, about 16 hours a week, spread across three days. • Fully remote, with quarterly visits in San Jose. • Applications are reviewed on a rolling basis.
