Case Manager - intake and assessment (remote)

Summit Nest Community Partners is looking for a Case Manager. The job centres on intake and assessment for benefits enrolment, and it would suit someone who likes steady, practical work. We are small enough to know everyone's name. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for benefits enrolment. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with benefits enrolment. Working arrangement • Contract for nine months, with clear deliverables from the first week. • Remote-friendly, with quarterly visits in Philadelphia. • We reply to every applicant.
