Case Manager - intake and assessment (remote)

We are growing our team at Westbrook Granite Community Partners Group and adding a Case Manager whose main responsibility will be intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for client intake. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with client intake. Working arrangement • Part-time, about 16 hours a week, with flexible days. • Remote-friendly, with a monthly team day in El Paso. • We reply to every applicant.
