Case Manager - intake and assessment (remote)

Westbrook Thistle Early Learning is an early-learning provider in Minneapolis, and we need a Case Manager to lead our intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for meal programmes. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with meal programmes. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • Remote-friendly, with quarterly visits in Minneapolis. • We reply to every applicant.
