Case Manager - intake and assessment (remote)

Highbridge Harbor Care Services is a social-care provider serving older adults and people with disabilities around Denver. We are hiring a Case Manager to take on intake and assessment. We train and promote from within. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for visit scheduling. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, with flexible days. • Remote-friendly, with quarterly visits in Denver. • We reply to every applicant.
