Case Manager - intake and assessment (remote)

Fernwood Vista Community Partners runs community programmes across Nashville. We are hiring a Case Manager to take on intake and assessment. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for food distribution. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with food distribution. Working arrangement • Part-time, about 16 hours a week, with flexible days. • Fully remote, with quarterly visits in Nashville. • Applications are reviewed on a rolling basis.
