Case Manager - intake and assessment (remote)

Ironwood Orchard Health runs clinics and patient services around Anchorage. We are hiring a Case Manager to take on intake and assessment. You would join a small organisation of about 179 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for clinic supplies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with clinic supplies. Working arrangement • Contract for three months, with clear deliverables from the first week. • Fully remote, with a monthly team day in Anchorage. • We will tell you what to expect at each step of the hiring process.
