Case Manager - intake and assessment (remote)

We are growing our team at Ironwood Sable Learning Group and adding a Case Manager whose main responsibility will be intake and assessment. You would join a small organisation of about 141 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Fully remote, with an occasional day on site in Madison. • We will tell you what to expect at each step of the hiring process.
