Case Manager - intake and assessment (remote)

Redstone Bridge Care Services is a social-care provider serving older adults and people with disabilities around Indianapolis. We are hiring a Case Manager to take on intake and assessment. You would join a large organisation of about 758 people, working mostly from home. What you will do • Help with applications and appointments for visit scheduling. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • Remote-friendly, with an occasional day on site in Indianapolis. • Applications are reviewed on a rolling basis.
