Case Manager - intake and assessment (remote)

Harborlight Mill Care Services Partners provides in-home and community support in Birmingham. We are hiring a Case Manager to take on intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for client records. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with client records. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with a monthly team day in Birmingham. • We reply to every applicant.
