Case Manager - intake and assessment (remote)

Bright Quill Health Group provides primary and specialty care in Milwaukee. We are hiring a Case Manager to take on intake and assessment. Clinicians and administrators work side by side. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for patient scheduling. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • Fully remote, with an occasional day on site in Milwaukee. • We will tell you what to expect at each step of the hiring process.
