Case Manager - intake and assessment (remote)

Summit Maple Community Partners Group is a nonprofit in Denver supporting families with food, housing and job help, and we need a Case Manager to lead our intake and assessment. We are small enough to know everyone's name. What you will do • Help with applications and appointments for food distribution. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with food distribution. • Bilingual English and Spanish. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Remote-friendly, with quarterly visits in Denver. • We reply to every applicant.
