Case Manager - intake and assessment (remote)

Foxglove Thistle Learning Group is looking for a Case Manager. The job centres on intake and assessment for family communication, and it would suit someone who likes steady, practical work. We follow the school calendar and we like people who like learning. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with an occasional day on site in Madison. • Applications are reviewed on a rolling basis.
