Case Manager - intake and assessment (remote)

Ironwood West Care Services is looking for a Case Manager. The job centres on intake and assessment for care plans, and it would suit someone who likes steady, practical work. The work is personal and so is the culture. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for care plans. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Full-time, Monday to Friday. • Remote-friendly, with quarterly visits in Anchorage. • We reply to every applicant.
